On Boarding

New Hire Forms

Prior to the initial day of employment, the following information should be forwarded to the Business Service Center:

  • Criminal History Check Authorization Form completed and signed by applicant
  • Written Offer Letter
  • Payroll Authorization Form (HRM-6 or HRM-8)
  • Documentation of at least two employment verifications
  • Official transcript(s) of highest degree earned or hours completed (if required for position OR was a selection factor for the vacancy).
  • Certification documentation – if a professional certification or affiliation is either a required qualification or a selection factor for the vacancy.

Salaried New Hire Forms

The Salaried New Hire Forms should be completed and forwarded within the first three days of employment for adjuncts who are less than 100% FTE and all other salaried positions that are not required to meet with HR Services.


Hourly New Hire Forms

The Hourly New Hire Forms should be completed and forwarded within the first three days of employment to the Career Center.

All non-student hourly employees’ forms should be forwarded to the Business Service Center.

If the hourly employee is a minor, the following form should be completed and forwarded with above paperwork:

Parent/Guardian Consent for Employment of Minor

If a student is a foreign national gainfully employed with the University of North Texas, it is a requirement to complete the Foreign National Information Form to evaluate the foreign national’s U.S. tax status (e.g. eligibility for exemption from income or FICA taxes, etc.) This form must be forwarded to the Business Service Center upon completion.


New Hire Information

The Salaried New Hire Informational Packet contains all of the required information that we must provide salaried new hires upon employment. This packet should be printed and provided to the employee to review and for future reference.

The Hourly New Hire Informational Packet contains all of the required information that we must provide hourly new hires upon employment. This packet should be printed and provided to the employee to review and for future reference.


Starting a new job is an exciting time for new hires. It's imperative that departments thoughtfully prepare to ensure new employees are comfortable in their new role and have the resources they need to successfully engage.
